The brief
Every order, result and bill attaches to one patient record. When it lands on the wrong one, the mistake follows the patient into the lab, the pharmacy and the next visit. Busy desks read a name out and wait for a yes, open the first search result, or click merge when the system suggests it. The course needed one habit that takes seconds and holds under pressure.
What learners can do after
- Ask the patient for their full name and date of birth instead of reading them out
- Match two identifiers before opening any record
- Never use a room or bed number as an identifier
- Stop and flag possible duplicate records instead of merging them
- Confirm both identifiers on the wristband and lab order with the patient
The framework
Three checks, every patient, every time. Two identifiers carry through all three.
-
1
Ask
Ask for full name and date of birth. Don't read them out and wait for a yes.
-
2
Match
Match both identifiers before opening a record. Not the first result, not a room number.
-
3
Confirm
Check the wristband and order against the record, then read them back with the patient.
How we built it
One check-in runs the whole course. Grace Mendoza is here for blood work, and the search returns two records that share her name and birthday; only the year differs. Learners watch Nicole ask instead of read out, match before she opens anything, stop at the merge prompt and flag both records, then read the wristband back with Grace before it goes on.
It continues ClearPath's series in the same look as Handling Patient Information Safely, built from the same screen kit and character set, so a second module costs a fraction of the first.
